[math-ias] IAS Flu Shot Clinic - Thursday Oct. 10, 9-11 a.m.

On Thursday, October 10, between 9:00 and 11:00 AM, a registered nurse
from Passport Health will be administering influenza vaccinations in the
Dilworth Room.  Adult members of our community who have pre-registered
with Human Resources prior to October 7 may participate.  The cost of the
vaccination this year will be $20, payable by cash or check written to
“IAS”.  As an alternative, you may be able to secure your influenza
vaccination at no cost through your primary care physician and you should
contact your doctor’s office directly for more information.  If you are
enrolled in one of our Aetna plans, receipts will be available which can
be submitted directly to Aetna for reimbursement.

The 2013-2014 trivalent influenza vaccine is made from the following three
viruses: an A/California/7/2009 (H1N1)pdm09-like virus, an A(H3N2) virus
antigenically like the cell-propagated prototype virus
A/Victoria/361/2011, and a B/Massachusetts/2/2012-like virus.  These are
the three main flu strains that research indicates will cause the most
illness during the coming flu season.

Attached to this memo is an information sheet prepared by the Department
of Health and Human Services’ Centers for Disease Control and Prevention
that should answer most questions about the vaccine.  In general, you
should speak with your personal physician before signing up for this
program if you have a serious allergy to eggs, have had a serious allergic
reaction after getting an influenza vaccine in the past, were ever
paralyzed by Guillain-Barré Syndrome, or have a moderate or severe illness
on the day of the inoculation.

Also attached is a consent form.  You can fill this one out and bring it
with you or there will be forms available at the clinic.

On the day of the inoculation, you should wear a loose-sleeved or
short-sleeved shirt.  If the time and date are inconvenient for you, you
can call Passport Health directly at 732-978-4792 to make an appointment
to be inoculated at their office located at 132 Franklin Corner Road,
Lawrenceville.  The cost to receive the vaccination at the Passport Health
office will be $25.

This program is open to all faculty, staff and members and the adult
members of their immediate families.  To participate, pre-registration and
pre-payment is required.  Please e-mail Human Resources at hr at ias.edu or
leave a message at 609-734-8040 prior to October 7 to register for a time
slot.  We group vaccinations in time slots scheduled every 15 minutes from
9 AM to 11 AM so please indicate your time slot preference and someone
will contact you to confirm.  Payment will be collected in Human
Resources, Fuld Hall, Office 102.
